Selenium does not float freely. It is incorporated into about 25 selenoproteins, including glutathione peroxidases (GPx1 through GPx4) and the iodothyronine deiodinases (DIO1, DIO2, DIO3) that convert inactive T4 into active T3 in your thyroid, liver, and kidneys. Why Women Are at Higher Risk of Deficiency

What Does a Low Selenium Level Mean?
A serum selenium below 70 mcg/L signals that your body's selenoprotein machinery is likely running below optimal capacity. Severe deficiency (below roughly 40 mcg/L) is associated with Keshan disease, a cardiomyopathy historically seen in selenium-poor regions of China, but this level is rarely seen in women in North America or Europe who eat a varied diet.
More clinically common for women in these regions is a functional low: a level between 70 and 90 mcg/L that may blunt thyroid hormone conversion and antioxidant enzyme activity without producing overt symptoms.
Conditions That Deplete Selenium
- Inflammatory bowel disease (Crohn's, ulcerative colitis): impaired absorption is well documented.

Symptoms of Low Selenium
Low selenium does not announce itself with dramatic symptoms. You may notice:

What Does a High Selenium Level Mean?
A serum level above 150 mcg/L indicates excess exposure. Selenium toxicity, called selenosis, typically appears at sustained intakes above the 400 mcg/day UL. The most common cause in women in high-income countries is over-supplementation: a daily multivitamin (often containing 55 to 100 mcg) combined with a separate selenium capsule (often 200 mcg) and regular Brazil nut consumption can push total intake well past the UL.
Early signs of selenosis include:
- Garlic-odor breath (dimethyl selenide excretion through the lungs)
- Brittle nails that break or detach
- Hair loss, paradoxically the same symptom as deficiency
- Nausea and gastrointestinal distress
- Fatigue and irritability
The Nutritional Prevention of Cancer (NPC) trial used 200 mcg/day of selenium-enriched yeast and raised baseline levels from roughly 113 to 190 mcg/L, a level above the upper reference limit. The subsequent SELECT trial (Selenium and Vitamin E Cancer Prevention Trial), which enrolled over 35,000 men, found that selenium supplementation at 200 mcg/day actually increased type 2 diabetes risk by approximately 7 percent in men who entered the trial with already-adequate selenium levels. Although SELECT enrolled men, the diabetes signal has been replicated in at least one analysis of women and is the main reason clinicians now discourage routine high-dose selenium supplementation without a documented deficiency.
Evidence-Based Ways to Raise a Low Selenium Level
If your serum selenium is below 70 mcg/L, the goal is to bring it to the 80 to 120 mcg/L range, which appears to be the functional sweet spot for selenoprotein saturation without accumulation risk.
Dietary Sources: The Food-First Approach
Food-derived selenium is predominantly in the form of selenomethionine, which is absorbed at roughly 90 percent efficiency from plant and animal sources. This is the form your body handles most gently.

Supplementation: Dose and Form
For women with a confirmed serum level below 70 mcg/L, most clinicians use selenomethionine or selenium-enriched yeast at 100 to 200 mcg/day for 12 to 16 weeks, then recheck levels. Sodium selenite is an inorganic form with slightly lower bioavailability and a narrower margin before toxicity.

- Perimenopause and post-menopause: Thyroid autoimmunity peaks in the perimenopause transition. Checking serum selenium alongside a full thyroid panel (TSH, free T4, free T3, TPO-Ab) is clinically sensible at this stage if fatigue, hair thinning, or antibody elevation is present. Supplementation at 100 mcg/day is a conservative starting point.
- Postpartum thyroiditis risk: Women who carry TPO-Ab in the first trimester have a 30 to 50 percent risk of developing postpartum thyroiditis. A 2007 meta-analysis in the Journal of Clinical Endocrinology and Metabolism found that selenium supplementation during pregnancy and the early postpartum period reduced postpartum thyroiditis incidence and TPO-Ab levels in antibody-positive women.
- Dietary restriction or bariatric surgery: Work with a registered dietitian and use selenomethionine 100 to 200 mcg/day long term; recheck annually.
Optimizing Absorption
Selenium absorption is generally efficient regardless of meal timing. The practical tips that matter:
- Do not take selenium at the same time as calcium carbonate supplements; calcium may modestly impair non-heme mineral absorption.
- Iron deficiency co-exists with selenium deficiency in women more often than in men; correct both because iron-deficiency anemia can itself impair thyroid hormone synthesis independently.
- Vitamin E (as d-alpha-tocopherol) and selenium have synergistic antioxidant activity, though supplementing high-dose vitamin E without documented deficiency is not recommended based on SELECT data.
Evidence-Based Ways to Lower a High Selenium Level
If your level is above 150 mcg/L, the intervention is almost always removal of the source, not treatment of the element itself.
Step-by-step approach:
- Audit your supplements. Count selenium from your multivitamin, any thyroid-support blends (many contain 100 to 200 mcg), separate selenium capsules, and Brazil nut intake. Total everything.
- Stop dedicated selenium supplements if your dietary intake alone exceeds 55 mcg/day, which it does for most women eating animal protein.
- Reduce Brazil nut intake to two or fewer nuts per week if you were eating them daily.
- Recheck serum selenium in 8 weeks. The half-life of selenium in the body is approximately 100 days, so normalization takes time.
- If levels are above 250 mcg/L or you have overt selenosis symptoms, refer to a physician for evaluation; selenium does not have a pharmacological antidote, and management is supportive.
Thyroid-support supplements sold to women are a particularly common hidden source. A 2013 JAMA Internal Medicine analysis of thyroid supplements found wide variability in actual selenium content versus labeled amounts, and several products exceeded 200 mcg per dose. Read labels carefully.
Selenium Across Female-Specific Conditions
Hashimoto's Thyroiditis
This is where selenium evidence in women is strongest. The 2002 Gärtner RCT and multiple subsequent trials show TPO-Ab reduction with 200 mcg/day of selenomethionine or selenite. The American Thyroid Association does not yet include a formal selenium recommendation in its 2022 guidelines because the evidence for hard outcomes (reduced progression to overt hypothyroidism) remains mixed, but most thyroid specialists consider a 3 to 6-month trial reasonable in TPO-Ab-positive women with serum selenium <90 mcg/L.
PCOS
Women with PCOS show both higher systemic oxidative stress and lower serum selenium than controls in multiple cross-sectional studies. A 2018 clinical trial published in Hormone and Metabolic Research enrolled 64 women with PCOS and found that 200 mcg/day selenium for 8 weeks significantly reduced serum insulin, HOMA-IR, and inflammatory markers compared with placebo, without changing BMI. This is a promising finding but the trial was small; replications in larger cohorts are needed.
Fertility and Preconception
Selenoproteins, particularly GPx4, protect sperm from oxidative damage. In women, GPx4 is expressed in the oocyte and early embryo, where it defends against lipid peroxidation. Low selenium in the follicular fluid has been associated with poor oocyte quality in some observational studies, though randomized trial data in women undergoing IVF are sparse. For women actively trying to conceive, meeting the RDA of 55 mcg/day through food is sensible; high-dose supplementation beyond 200 mcg/day has not been shown to improve live birth rates and carries the diabetes and toxicity risks described above.
Postpartum Thyroiditis
Women who are TPO-Ab positive before or during pregnancy carry elevated risk of postpartum thyroiditis, which affects approximately 5 to 10 percent of all postpartum women. The 2007 meta-analysis by Toulis et al. In JCEM reported that selenium supplementation in antibody-positive pregnant women reduced the incidence of postpartum thyroiditis at 3 months (OR 0.29, 95% CI 0.14 to 0.61). This finding has influenced practice but has not yet been incorporated as a standard-of-care recommendation by ACOG or The Menopause Society, partly because of the evidence gap in large, adequately powered RCTs.
Pregnancy, Lactation, and Selenium: What You Need to Know
Selenium is not a teratogen at dietary doses. It is an essential nutrient with increased requirements during pregnancy and lactation.
Pregnancy RDA: 60 mcg/day. Most women in North America who eat animal protein meet this through diet. Women following strict vegan diets in low-selenium-soil regions (parts of the Pacific Northwest, New England, and most of Europe) may need supplementation.
Lactation RDA: 70 mcg/day. Breast milk contains approximately 15 to 20 mcg/L of selenium, and production of roughly 750 mL/day exports 11 to 15 mcg of selenium daily from the mother. If you are breastfeeding and your diet is selenium-poor, depletion is possible over months of exclusive nursing.
High-dose selenium in pregnancy: use caution. Animal data show that doses well above the UL are teratogenic in rodent models. Human data at the UL (400 mcg/day) are not available in pregnant women, and this dose should be avoided. Prenatal vitamins typically contain 20 to 70 mcg of selenium; adding a separate 200 mcg selenium supplement without a documented deficiency is not recommended during pregnancy.
Supplementation if TPO-Ab positive: As noted in the Toulis meta-analysis, 200 mcg/day selenomethionine in TPO-Ab-positive pregnant women appears to reduce postpartum thyroiditis risk and is generally considered safe at this dose during pregnancy, though it should be done in consultation with your OB-GYN or maternal-fetal medicine specialist rather than self-prescribed.
Contraception note: Selenium supplementation has no known teratogenic risk at dietary or near-RDA doses and does not require contraception. High-dose supplementation (>400 mcg/day) should be avoided if you are pregnant or planning pregnancy due to the lack of human safety data at those levels.
Who Should and Should Not Supplement Selenium
Good candidates for a monitored selenium supplement trial:
- Women with confirmed serum selenium <70 mcg/L
